Medical Creator is the complete ambulance job and hospital system for FiveM. Players who go down enter a last stand where they can crawl and call for help, EMS arrive with a radial menu, diagnose every body part on a live injury report, treat bleeding, bruising and burns with real items, perform a CPR revive and carry the patient out on a stretcher. Nobody available? The patient checks in at the hospital NPC, is laid on a free bed and recovers on camera.
What sets it apart is the in-game hospital creator. Every hospital — its area, blip, respawn point, check-in NPC, beds, duty point, stash, boss menu, wardrobe, pharmacy and garages — is drawn and placed in game with a free camera, saved to the database and synced to every player instantly. It runs on ESX, QBCore and Qbox, detects your inventory, target, keys, fuel, clothing, billing and dispatch on its own, and ships in 27 languages.

What it does#
Area | Details |
|---|---|
Hospital creator | Unlimited hospitals drawn in game: a polygon area, blip, respawn point, check-in NPC with beds, duty, stash, boss menu, wardrobe, NPC pharmacy and any number of garages with vehicles per job grade. |
Death & last stand | A downed phase where the player crawls, a death screen with a countdown, a distress call to EMS, bleed-out respawn, a drowning timer, and death that survives a reconnect. |
Injury system | Six body regions, three injury types (bleeding, bruising, burning), four levels each — with limping, blurred vision, weaker aim, blood particles and health loss over time. |
Treatment | A live injury report over the patient's body: drag tweezers, an ice pack or burn cream onto each injured part. Optional rule: no revive until every wound is treated. |
EMS menu | A hexagon radial menu on F5: diagnose, CPR revive with a defibrillator, heal, sedate, put in and out of a vehicle, bill, dispatch calls, stretcher and wheelchair. |
Check-in & beds | Pay at the doctor NPC, get laid on a free bed and recover on camera. Only available while fewer than a set number of EMS are on duty, with an insurance discount. |
Stretcher & wheelchair | A carryable stretcher that holds a patient and loads into the ambulance, and a streamed wheelchair vehicle a doctor prescribes for a set time. |
Medications | Painkillers, morphine, hemostatic agent, burn gel, adrenaline, antibiotics, epinephrine and sedatives — each with a duration and real side effects — plus a medical bag EMS drop on scene. |
Logs | Discord webhooks for deaths (killer, weapon, distance), revives, heals and hospital edits, with optional screenshots through FiveManage. |
Languages | 27 translations, from English, Spanish, French, German and Portuguese to Arabic, Japanese, Korean and Chinese. |

Driving it from your own scripts? Every player's state is published as
Player(id).state.dead, and one event revives or heals anyone. See Commands and Exports.
